STOCKHOLM -- Sweden's chief epidemiologist showed contrition Wednesday as criticism mounted over the Scandinavian country's hotly debated method of fighting the coronavirus, which has resulted in one of the highest death rates per capita in the world.


Sweden has stood out among European nations and the world for the way it has handled the pandemic, not shutting down the country or the economy like others but relying on citizens' sense of civic duty. Swedish authorities have advised people to practice social distancing, but schools, bars and restaurants have been kept open the entire time. Only gatherings of more than 50 people have been banned.


"I think there is potential for improvement in what we have done in Sweden, quite clearly," Anders Tegnell of the Public Health Agency told Swedish radio.


Sweden, a nation of 10.2 million people, has seen 4,468 deaths linked to COVID-19, which is far more than its Nordic neighbours and one of the highest death rates per capita in the world. Denmark has had 580 coronavirus deaths, Finland has seen 320 and Norway has had 237, according to a tally by Johns Hopkins University.


"If we were to encounter the same disease again, knowing precisely what we know about it today, I think we would settle on doing something in between what Sweden did and what the rest of the world has done," said Tegnell, considered the architect of the unique Swedish pandemic approach.


Authorities in Sweden, including Tegnell, have been criticized -- and have apologized -- for failing to protect the country's elderly and nursing home residents.

But Tegnell said Wednesday it was still unclear what the country should have done differently. He also said other nations are unable to tell exactly what measures affected the outcome of their outbreaks because they threw everything at it in one go.


"Maybe we know that now, when you start easing the measures, we could get some kind of lesson about what else, besides what we did, you could do without a total shutdown," Tegnell said.


Asked if the country's high death toll has made him reconsider his unique approach to the pandemic, Tegnell answered "yes, absolutely."


"I'm not walking around thinking that we have a real disaster here in Sweden," Jan Arpi, a 58-year-old sales executive, told The Associated Press. "I think we have it more or less under control, but we have to be even more careful now with the learning we have got from how the virus is spread, especially among the elderly people,"


Sweden's infection rate is 43.24 deaths per 100,000 inhabitants is lower than Spain's (58.06), and Italy's (55.39), but is higher than the reported rates in the United States (32.14) and Brazil (14.29), according to the Johns Hopkins University.


Last week, the country's former state epidemiologist, Annika Linde, said that in retrospect she believes an early lockdown could have saved lives while political pressure has forced the government to bring forward an investigation into the handling of the crisis.


The moves recommended by Tegnell have made Sweden a bit of a local pariah and didn't spare the Swedish economy. More than 76,000 people have been made redundant since the outbreak began and unemployment, which now stands at 7.9%, is expected to climb higher.


Finance Minister Magdalena Andersson has said Sweden's economy, which relies heavily on exports, will shrink 7% in 2020 and the Scandinavian country was headed for "a very deep economic crisis."


Last week, neighbouring Norway and Denmark said they were dropping mutual border controls but would keep Sweden out of a Nordic "travel bubble."


Danes said they will reopen the border next month to residents of neighbouring Germany, as well as to Norway and Iceland, as it accelerates the easing of its coronavirus lockdown. However, Denmark, which has a bridge that goes directly to Sweden, has postponed a decision on whether to reopen to Swedish visitors until after the summer.

Two of Sweden’s top ministers have defended the country’s decision not to impose a lockdown in response to the coronavirus pandemic and insisted this approach had strong public support, despite a mounting death toll and sharp criticism from some scientists.

Deputy prime minister Isabella Lovin told the Financial Times that Sweden had taken “very harsh and exceptional measures”, despite keeping its schools and restaurants open, as it wanted “to take the right decisions at the right time”.

Ann Linde, foreign minister, said in a separate interview: “We don’t believe in a lockdown if it’s not going to be sustainable over time. We don’t believe we can lock people in their houses for several months and have a high degree of people following it. But it’s a myth that it’s business as usual. It’s not business as usual.”

Sweden, which is an international outlier in resisting a lockdown, has suffered a much higher death toll than its Nordic neighbours, even adjusting for its population being roughly double that of Norway, Denmark or Finland. As of Wednesday, Sweden had 1,203 deaths due to coronavirus, against 143 for Norway, 309 for Denmark and 72 for Finland.

Swedish authorities are under pressure due to a rising number of coronavirus infections in elderly homes. Anders Tegnell, the state epidemiologist, noted this week that the average age of those dying in Sweden was markedly higher than in Norway. “It is not a failure for the overall strategy, but it is a failure to protect our elderly who live in care homes,” he added. Ms Lovin called it “very worrying” and added that ministers were calling municipalities with few infections to urge them to protect the elderly.

Both Ms Lovin and Ms Linde underlined that Sweden was following its tradition of “more than 100 years” of following the advice of expert authorities when making decisions. Sweden’s public health agency recommended against closing primary and secondary schools, but the country did close them for students older than 16.

Denmark reopened its kindergartens and primary schools up to fourth grade on Wednesday, with Norway set to follow in the coming two weeks. But the governments in both countries decided not to follow the advice of their respective health agencies to have a broader opening of the school system.

More than 20 Swedish epidemiologists and scientists published an article this week urging politicians to take over from the public health agency and shut down schools and restaurants to avoid Sweden following the same path as Italy in terms of coronavirus deaths. “The approach must be changed radically and quickly,” they wrote.

Ms Linde said the scientists were “entitled to their opinion”. But she added: “It’s very different to compare countries with different circumstances and different stages of the virus. We are all fighting the same fight but with different means.”

Much of the Swedish approach is centred around the idea that dealing with coronavirus will be “a marathon, not a sprint”, with measures likely to be in place for months, if not years. Authorities believed that closing schools and kindergartens would have caused some key workers such as doctors and nurses to have to stay at home — or for at-risk grandparents to be drafted in to do childcare.

Ms Lovin said that Sweden had tried to avoid legal restrictions where possible but had left people in no doubt as to what was recommended, as combating Covid-19 demanded a “common responsibility”. She pointed to travel at Easter — when the government exhorted Swedes not to travel to their summer cottages or relatives, but did not ban them from doing so — which was 90 per cent lower than in previous years.

“Our understanding of this epidemic is that it will be with us for a very long time. It’s very important that we have sustainability in the decisions we take so that we don’t have a fatigue in the population. There’s huge support for the way we have been doing it so far. There is almost no controversy between the political parties,” she added, noting one query from the populist Sweden Democrats as to why schools were still open.

Swedish business people have been perhaps the most vocal in Europe on the need for authorities to balance the needs of the economy and plan for an eventual recovery with the measures required to fight coronavirus. Ms Lovin, co-head of the Green party, said “the recovery needs to be a green recovery”, adding that she was worried that “as after the financial crisis that countries and companies are so keen to restart the economy that they throw all the green and sustainable aspects overboard”.

      I agree that this real-time experiment with the world’s population is fascinating and will undoubtedly produce very interesting results. Having magically rediscovered the value of borders (recently believed to be worthless, imaginary constructs), nations have now become laboratories testing different theories about and approaches to the Wuhan virus as well as likely revealing different ethnic and racial susceptibilities to the thing. There is even growing suspicion that the virus itself is lab-born. Stay tuned indeed and pray we’re the guinea pigs in the right lab.

      Huh? --This is the correct answer from someone who neither understood my observation nor ever seems capable of thinking outside the box. Sweden "flaunting protocol"? Whose protocol? "Blowing it" ? Says who? The story is far from over. We will see. So far they are under the curve. This article set out with the agenda I pointed out and did not offer a comprehensive contextual picture which is the stock in trade of today's media. Your reflexive anti Trump comments only augment my observation. I won't cheer against Sweden like the sick media is now doing. I'll watch and see. So far they are far from falling off a cliff because they took a lighter approach. These are things called facts, ungoverned by political identity or mindless group think.

      Without measures taken 1/2 million dead? We don't know that. That's now Trump's narrative. And it will be the narrative everywhere else too. The whole point is that Sweden's experience may answer the question one way or the other. This is not difficult. The only non sense is in your head. You really should exercise your god given reason and stop assuming that insulting people makes you smart.

      Yes they are harsh. The worst part is no plan for a quicker opening of business which is the backbone of our economy. Trudeau simply states it will be weeks until anything changes and our economy does not have weeks to sit idly on the sidelines. All businesses can be open now with social distancing masks.

      Again these statistics are potentially misleading. The average age of death in Italy form COVID-19 is 79.5 years. The loss of years of life in Italy is on average 3 which means the median number is probably close to 1. My question to you Ivan is how much do you think we should spend as a society to extend life by a year? My ball park estimate is that it is costing us about $1,000,000. What if we can get the number of deaths from COVID-19 down to a very low level at the cost of $10,000,000 per death. Where is the sweet spot for you?

疫情冲击下,逆全球化与产业链转移的讨论空前火热,各国高层陆续开始放话。3月5日,安倍在“未来投资会议”中表示要让高度依赖一国且高附加值的产业回国,让低附加值的产业向东盟转移,以此分散风险。4月9日,白宫首席经济顾问库德洛招揽在外美企回国,声称企业搬迁政府买单。

外部环境恶化,身为世界工厂的中国被推上风口浪尖。然而谈起产业转移,还有一个绕不过的东南亚小国:越南。

放眼整个东南亚,越南发展速度惊人,且与中国的发展历程有高度相似之处,都是得益于锐意改革与积极入世,享受全球化的红利。本次疫情影响下全球需求放缓,越南的制造业PMI从50%下坠到30.7%,手机、服装鞋类等出口也大幅受挫,侧面证明了越南在全球化中的融入程度。

一个时常被忽视的事实是:越南从来就不是什幺小国。它的国土面积与日本相当,人口接近一亿,这个体量放在欧洲是不折不扣的大国。2019年年初,越南总理就定下越南2045年成为发达国家的目标,比我国的社会主义现代化还早了5年。

不过,越南的野心显然不止于经济,地理上紧临南海的越南长期斡旋于中美两国之间,“中国人民的老朋友”基辛格更是直接指出,越南的野心是成为东南亚的队长。



复杂的政治博弈我们暂且不讨论,聚焦于产业雁阵转移,随着投资越南的热度一浪高过一浪,一个重要问题也呼之欲出:越南,会是下一个中国吗?

01 中国与越南,同与不同

1986年,中国改革开放8年之后,越南见贤思齐,提出对应的“革新开放”政策,开启了市场化进程。三十年来越南经济迅猛发展,2018年GDP同比增长7.08%,创下过去十年新高,在全球经济增速放缓的背景下表现出众。

从数据上看,2018年越南人均GDP为2564美元,处于中国2007年前后的水平;2018年城镇化率为35.7%,仅是中国2000年前后的水平;2018年房地产投资占GDP比重仅为2.4%,中国早在1997年该比例就达到了5%;2014年越南人均耗电量为1424千瓦时,接近中国2003年的用电水平。


众所周知,2000年前后正是中国制造业和房地产迈向高速扩张的“前夜”,大规模产业链转移和外资涌入,当前的越南,似乎正在走我们过去走过的那条路。

越南是典型的外向型经济,贸易和外资是越南发展的重要引擎。上世纪90年代以来,越南进出口贸易额在GDP的占比除了2009年受到金融危机冲击有所下降外一路上涨,2018年更是高达200%。外资投入上,从2010-2019年,制造业FDI复合增速达到17%,截止2019年制造业吸收FDI占全部FDI比重已经达到65%[1]。

众多相似之外,两国在许多方面同样存在较大差异。具体看中越两国的产业结构可以发现一个有趣的现象:中国从2000年以来二产、三产比例变化明显,尤其是第三产业一直保持增长,但越南三大产业的结构并没有发生太大变化,尤其二、三产业的比重,表现出惊人的稳定性。这是为何?


储蓄率稳定是一大重要原因。不同于中国是个高储蓄率国家(近年来在持续下降),越南的储蓄率非常低,居民部门中储蓄占据收入的比重仅为25%,很难再下降,75%用于消费,已是接近美国的水平。受此影响,越南的消费马力十分强劲,在GDP的占比中高达80%,远高于投资。

不仅国内消费稳定,由旅游业发展带动的商贸、餐饮服务等同样支撑着第三产业,制造业转移又持续推动第二产业发展,使越南三产结构保持相对稳定。

除了产业结构不同,越南在部分环节表现上也与中国走出了趋势性的差距。从基础设施投资占GDP比重可以看出,随着时间推移,中越从2008年前后的相似水平走向两个方向,中国持续攀升至21.1%,而越南一路下降到7.9%。今日越南落后的基础设施建设状态,是越南亟需补齐的一大短板。



02 越南发展的四块基石

说起越南,我们常以为产业链转移越南最大的原因是人口红利,而在一份中金实地调查报告中发现,人口红利并不是海外投资者进入越南的首要原因,政局稳定和开放格局才是位列前二[2]。

1)政局稳定,以发展为先

越南政局稳定,以发展为先。财政积极为中小企业减税降费;银行放贷优先支持贸易、工业企业;实施外资优惠政策,支持越南招商引资。工业园区建设方面,截止2018年在相当于我国长三角城市群大小的国土面积上打造了326个园区,合计引入1.55万个项目,其中外资项目达到8500个。

一系列多角度全方位的政策中可以看到,越南深谙发展才是硬道理。

2)积极开放,拥抱全球化

21世纪以来,越南积极开放不断融入国际组织。1995年越南加入东盟,成为其融入地区经济合作的里程碑;1998年加入APEC,将合作关系从区域经济拓展到国际社会;2006年加入WTO,在贸易、投资、技术上打开新局面;2019年CPTPP生效,多边贸易合作再进一步。

在外汇管理上,越南自1989年金融体系改革以后,实行浮动汇率制,并且将币值稳定作为重要的货币政策目标,为外向型企业的发展创造稳定的后方条件。

3)人口四大红利

人口红利从人口数量红利、人口结构红利、人口质量红利、劳动力成本四个方面展开分析。

从人口数量看,上世纪八十年代以来越南政局稳定、经济逐渐发展,总人口以及其中的劳动力人口也随之不断增长,2018年人口总量达到9467万。不过越南总人口增长率已从八十年代初的2%缓慢滑落到如今的1%,劳动力增长率在2015年以前始终高于人口增长率,且在1990年和2012年出现过两个小高峰。

越南的人口数量红利仍在存在,但是已经出现拐点。适龄劳动人口占总人口比重在2012年前后出现下降趋势,抚养比也在2012年前后开始上升。


城镇化过程、产业结构转化释放了越南人口结构红利。2005-2017年间,越南的农业人口逐渐下降,制造业人口逐渐上升。且随着城镇化的不断推进,这一劳动力的结构转换将持续发生,释放人口红利。此外,随着互联网和智能手机的不断普及,制造业的劳动力部分将流出转向第三产业。


从人口质量看,越南平均受教育水平相对较高,15岁及以上成人识字率达到93.5%,虽然次于菲律宾、印尼、中国和马来西亚,但是差距比较微弱。而且越南科教文卫投资增速较快,人口质量红利未来还有较大发展空间。


劳动力成本优势更是明显,越南人均GDP虽然逐年增长,但是同其他东南亚国家相比仍处于低水平,尤其相较于马来西亚、中国和泰国6000至9000美元的人工成本,越南不到2000美元的人工显得尤其便宜。


4)地理优势

越南东面狭长的海岸线送来了优质的港口资源,例如越南北部的海防港和南部的胡志明港,成为开展贸易的一大超级王牌,许多东南亚国家并不具备如此优越的地理优势。

港口优势下,南部的胡志明市是越南的经济中心,拥有全国最大的港口和交通枢纽,面积2095平方千米,人口超过600万,对标中国的上海;北部的海防为越南北部最大港口城市,是首都河内的输出港,对标中国的天津;岘港位于越南中部,濒临南海,是重要的工业城市和海港。

03雁阵转移,东亚之路

越南制造业的快速发展有目共睹,制造业增加值占GDP的比重也在不断加大。产量方面,诸如洗衣机、手机产量的年均增长率可以达到24%和30%,服装、鞋类产量的年均增长率也可以达到9%和5%。越南制造业业绩表现亮眼,吸引的FDI投资增速也高于整体,二者相辅相成。

近两年宣布在越南投资建厂的企业不在少数,如下表所示,国家涉及美日韩以及中国,行业涉及芯片、屏幕等高科技产品,也涉及纺织、鞋业等中低端产品。


2018年毛衣战影响下,美企代工厂逐渐分散生产据点,降低对中国的依赖程度,越南则闷声发大财,承接了不少企业,我们以苹果产业链为代表一窥产业转移风潮。

苹果最大代工厂富士康2018年底便与越南河内市政府讨论在越建立新的iPhone制造工厂,以减轻毛衣战的影响;2019年8月,富士康在越南购置土地建设厂房,新建越南工厂。

歌尔声学在苹果供应链中以代工AirPods为主,2019年1月,歌尔声学将在中国的AirPods生产部分转移到越南,并在越南北宁省桂武工业区投资2.6亿美元[3]。

立讯精密是国内最大的连接器生产商,也是苹果的主力供应商。2019年7月立讯精密公告对外投资的两个动作:一是在越南义安投资7000万美元建设立讯精密(义安)有限公司;二是对“越南立讯”增资不超过1.8亿美元,以越南立讯为主体进行智能可穿戴设备产品的产能扩建。

从产业转移看,我们看到越南承接产业类型上与过去中国的高度相似性,现在的中国正在冲破荆棘走产业向上之路,本土企业的力量变得愈发关键,而越南的本土力量还在承受外资的冲击。

2013年越南本土企业占工业产出比例低至49.9%,近年来大量外资设厂,比例显然在进一步降低。越南本土企业占出口的比例表现愈发明显,2018年已经达到了过去20年的最低点,2018年仅为29.5%。中国本土企业力量则在2006年达到最低点以后一步步抬升。


相较之下,越南仍然任重道远。从各个指标对比上看,越南在承接产业转移上已是一个“低配版”中国。如今,中国已经向自主品牌与制造业崛起发力,显然,越南距此相去甚远。

   1.. The serum of the recovered patients is ineffective for treating patients in COVID-19.


   In Shanghai, China, for some patients whose virus cannot be eliminated, doctors treated by the serum of the cured person and some of the patients did turn negative, but they regained positive after three days.


   2.. Need to learn the effect of hydroxychloroquine whether it is immunosuppressant or anti-virus


   26 May 2020, a report from China Ruxolitinib in treatment of severe coronavirus disease 2019 (COVID-19 said that ruxolitinib recipients had a numerically faster clinical improvement with significant chest CT improvement, a faster recovery from lymphopenia and favorable side-effect profile.


   There were reports that immunosuppressant effect of Ruxolitinib caused infections on several organs. But in this study, it effectively cured patents in Covid-19; which inspires us to learn the effect of hydroxychloroquine whether it is immunosuppressant or anti-virus.

   The clinical cases in many countries proved that the effect of hydroxychloroquine is negative than positive in the treatment of COVID-19 patients with killing more lives. Therefore WHO has paused the trial of hydroxychloroquine that listed as one of four medications in SOLIDARITY trial. But in China, the clinical cases show that the effect of hydroxychloroquine is more positive than negative in the treatment of COVID-19 patients with saved more lives.



   The radical different results from the use of same hydroxychloroquine stemmed from the radical difference in the medical guidance - philosophy.  


   In western countries, the medical philosophy is Reductionism that regards human body as a mechanical assembly of inanimate parts, in attempts to reveal the mechanism of life and disease with infinitely subdivided human body as micro-units as protein or genes. The treatment is in focusing on looking for killing with ignoring the fact that it is psychological factors in decisively affecting the development of diseases.


   Reductionism dogmatically tempts people addicted into so called scientific to have made simple issue too complicated to deal with. It poisoned people rejecting experience based medications, while makes all efforts to prove those drugs made by scientific way as worse than good.


   In China, the medical philosophy is the combination of Reductionism and Daoism of Traditional Chinese Medicine – TCM; which regards human body as organic whole with all organs mutually influencing of each other. According to available information, especially the attacking target, the arguments on the use of hydroxychloroquine may be triggered by the researches that led by French infectious disease expert Professor Didier Raoult, which proved that hydroxychloroquine is an effective treatment to cure COVID-19 patients in decreasing the virus carriage duration.

Abstract

BACKGROUND:

Chloroquine and hydroxychloroquine have been found to be efficient on SARS-CoV-2, and reported to be efficient in Chinese COV-19 patients. We evaluate the role of hydroxychloroquine on respiratory viral loads.

PATIENTS AND METHODS:

French Confirmed COVID-19 patients were included in a single arm protocol from early March to March 16th, to receive 600mg of hydroxychloroquine daily (200 mg, 3 times/day) and their viral load in nasopharyngeal swabs was tested daily in a hospital setting. Depending on their clinical presentation, azithromycin was added to the treatment. Untreated patients from another center and cases refusing the protocol were included as negative controls.
